How do I use my membership?
You can call up the primary care doctor that you have selected and make an appointment.
You have access during normal business hours for the doctors
Let the doctor's office know what Plan you are on.
You will need your name, DOB, and Plan
You will have 24-hour access if you picked the expedited processing.
If you did not pick the 24-hour expedited processing your membership begins on the 5th of the following month. Membership runs from the 5th to the 5th. If you signed up on the 1st through the 4th of the month, your membership would begin on the 5th of that month.

Dr. Todd Cramer: Monday and Wednesday morning 7:30 am to 11:00 am, afternoon 3:00 pm to 6:30 pm
Tuesday 3:00 to 6:30 pm, Thursday 3:00pm to 6:30pm, Friday 7:30 am to 11.00 am
Saturday by Appointment
Dr. Jerry Cramer maintains the same hours as above (except he is open on Friday all day, closed for lunch noon till 2pm)

Dr. Jacobsen: M-F 8 am to 5:30 pm

No, you can call the Doctor directly to make an appointment or the added value services that are part of your plan.
I need lab work, what do I do?
Contact your doctor for a lab slip; the provider may want you to make an appointment first in order to get a lab slip.
